    A single bounded task with the evidence required to complete it.

    Assign a risk owner / escalate

    Track ownership and evidence-based resolution.

Heads up — Creating a task does not close the parent exception. Accepting task evidence completes only that task; an unresolved funding decision keeps the exception open. Missing evidence is not a verified performance failure.
